STEP 1
COMPLETE & PRINT PASSPORT FORM LOCATED ON THE DEPT. OF STATE WEBSITE

STEP 2
GET YOUR PASSPORT PHOTOS!
Head over to your local Kinko’s or a place where Passport photos are taken.
STEP 3
VISIT A POST OFFICE WITH A PASSPORT OFFICE OR A PASSPORT AGENCY
You can locate your nearest Passport office HERE
PLEASE BRING:
- Printed Passport form (DS-11)
- Original Birth Certificate
- Passport photos
- Driver’s License or Gov’t issued ID
